In this paper, we present a heuristic model that accounts for the dynamic and interactive nature of structural factors that may impact HIV prevention behaviors. We demonstrate how structural factors influence health from multiple, often interconnected social levels and how, through the application of principles of systems theory, we can better understand the CPI-455 dose processes of change among social systems and their components. This model provides a way to delineate various structural intervention mechanisms, anticipate potential direct and mediated effects of structural factors on HIV-related behaviors, and provides a framework to evaluate structural interventions. We apply this model to two significant behaviors in HIV intervention as case illustrations, namely, HIV testing and safer injection facilities. Finally, we discuss ongoing challenges in the development and evaluation of structural interventions for HIV prevention, detection, and treatment.
